CHC’s Customer Rewards Program
CHC’s Customer Rewards Program is designed as an incentive for new customers to sign up with us and as a thank you to our loyal and long-time customers. A points system will be used, which allows you to qualify for smaller gifts fast or continue collecting points over time for more substantial gifts. The idea is not to entice you to use more agency, but to give you a reason to call us first when you do need agency.
Rewards Points will be based on the number of shifts booked, worked, and paid. The chart below shows how Rewards Points are earned:

Points will be calculated monthly by totaling the shifts on your weekly invoices. Points may be used for Rewards immediately upon timely payment of the invoice. Past due invoices will prevent eligibility for Rewards Points and Rewards. Rewards Points and Rewards may not be earned retroactively by paying past due invoices. We are asking our customers and potential customers for input on Rewards ideas that would be appropriate for their residents, but only Rewards on the CHC Rewards List at the time of points redemption may be selected.
The Program’s first term will be from February 1, 2024, through June 30, 2024. If the program is a success, we will extend the program through December 31, 2024. Customers may redeem their Rewards Points for Rewards after the program’s end date, but only invoices paid on time and by the end date will be eligible for Rewards Points and Rewards. All Rewards are offered as-is. Critical Health Connection disclaims all warranties (including merchantability and fitness for a particular purpose). Requesting, accepting, and/or using any item offered as a Reward assumes all risks associated with its use or misuse.
